Family Medical Services

Those without insurance may qualify for services offered on a sliding fee scale, based on income and family size. Proof of income must be provided at the first appointment. 

The Sliding Fee Discount Program (SFDP) is a program that may offer you a discount on your medical, dental and behavioral health charges. It sets a discount on what you pay based on the size of your family and how much money your family makes in a year.

Your level of discount is based on the Federal Poverty Guidelines established every year by the U.S. Department of Health and Human Services.
